Case Manager - Bridging Families Location: Crossnore, North Carolina Crossnore Communities for Children seeks Case Managers for our Crossnore, NC location.
This role will coordinate service delivery for children and families who are clients in Crossnore’s Bridging Families program by managing collaboration with the clients and their families, and other Bridging Families’ team members such as community representatives, legal guardians, and family members.
Hiring Range: $39,500 to $50,000 annually Pre-Hire Requirements: Must successfully complete extensive background checks, interviews, and pre-hire drug screen.
Crossnore Communities for Children is requiring new hires (excluding those with an approved medical or religious exemption) to be vaccinated for COVID 19 as a condition of employment to help ensure the safety of all employees, youth and families served.
Benefits: Defined Contribution Benefits Plan including options for: Health/Dental/Vision Insurance Life, Short-Term Disability, and Long-Term Disability plans Critical Illness and Accident coverage options Flexible Spending Plan 401k with employer match (up to 6%) Ongoing Training 20 Days PTO, 11 Paid Holidays JOB INFORMATION REPORTS TO: Bridging Families Supervisor STATUS: Full Time, Exempt QUALIFICATIONS Education/Experience Meets requirements for a Qualified Professional which are: Master's degree in a human services or related field from an accredited college or university and a minimum of one year supervised clinical experience working with children and families; or Bachelor's degree in a human services field and two years' experience with the current population; or Bachelor's degree in a non-human services field and four years' experience with the current population Two (2) years experience as a Case Manager or similar direct care position required.
Experience in residential programs, foster care, or youth mental health services preferred.
Skills/Competencies Working knowledge of the state mental health and child welfare system required.
Experience working with the team model approach in serving clients and families.
Experience in administrative functions; maintaining files and electronic medical records, case logs and process notes; organizational and monitoring skills.
Ability to be flexible and work adaptively and creatively in an interdisciplinary setting.
Ability to use sound discretion and independent judgment in developing and implementing decisions and recognizing circumstances when involving Supervisor/Program Leadership is necessary.
Ability to work well with others; flexible (including work days and schedules), ability to develop positive and professional relationships with children and their families.
Demonstrated proficiency in the use of a variety of computer software programs, including Google and Microsoft products.
Professional interpersonal, communication and organizational skills.
Must be able to balance and prioritize multiple tasks and/or projects.
Ability to communicate effectively and relate to staff, children, clients and the public in a positive and pleasant manner.
Demonstrated ability to operate calmly and decisively in crisis situations and knowledge of cultural issues that can affect care.

Ensure the provision of quality services through participation in Crossnore Communities for Children Performance Quality Improvement (PQI).
JOB DUTIES AND RESPONSIBILITIES Serve a caseload and establish rapport with clients/families/legal guardians to develop meaningful therapeutic relationships.
Coordinates, with all relevant team members, the development of and updates to service plans (person-centered plans) with clinically and developmentally appropriate life-skill goals for children served in the Bridging Families program.
This includes facilitating team meetings with internal staff and external stakeholders to develop goals that compliment a family/client’s goals and permanency plan.
In conjunction with other members of the treatment team, support the completion of assessments for all clients in the program.
Develop and maintain positive relationships and contact with school staff, family members, DSS, Crossnore staff, visiting resources, GAL’s and community resources to ensure good communication and teamwork.
Consult on a regular basis with collateral contacts, including school officials, additional providers, medical professionals, etc.
Including obtaining prior service records, medical records, and educational records.
Connect clients and their families to appropriate resources.
Facilitate barrier-free access to resources as appropriate in conjunction with the plan of care, including referrals to resources and transportation in many instances.
Ensure that all necessary documents are present in the electronic client file (birth certificate, social security card, insurance information, CBA’s and POC’s etc.
) Ensure timely acquisition of documents for the electronic file.
Provide court support, advocacy, and competent testimony as needed.
Assist with coordination and transportation of students as needed Coordinate requests for financial assistance for items such as automobiles, insurance, special camps or other special opportunities Ensure that adequate discharge planning is completed for all clients, especially those turning 18 years of age.
Discharge planning documents include the Discharge Form, Discharge Memorandum, Discharge Planning form and Discharge Summary Ensure that the foster care 18-21 year old programming for DSS students is utilized whenever possible and that it connects with the agency’s internal program for this age population.
Facilitate parent skill curriculums to instill parents with new skills.
Facilitate psycho-educational curriculum with families to increase knowledge about awareness and effects of trauma on adults and children.
Provide in-person coaching to assist in transfer of learning to day-to-day activities.
Coordinate appropriate transition step-down services for the family and children to meet ongoing needs post discharge.
Assist Bridge Parents in assuring all medical, dental, vision and mental health needs of children are met and assists families in accessing services to meet these needs.
Provides assistance to Bridge Parents/Residential Staff in aiding family in planning and implementing meaningful family time (visitation).
Assist with managing crisis situations, in conjunction with the entire Bridging Families team.
Submit Level 2 & 3 or Critical Incident Reports to the appropriate reporting party.
Follow up after Level 2, 3, or Critical Incidents in regards to recommendations for treatment services, obtaining additional support services, and communicating with contacting collateral contacts, such as Hospital staff, DSS Investigators, etc.
Regularly attend and facilitate client staffing and team meetings.
Track, monitor, and submit ongoing authorizations for services.
Conduct ongoing and follow-up surveys with each family served.
Responds to referral inquiries (phone, email, face to face, etc.
) regarding children/ families seeking Outpatient Therapy Services.
Partner with the referrals and admissions team to facilitate assessment and intake meetings for new clients, as assigned.
Complete miscellaneous tasks related to client care as assigned by their supervisor.
DAYS/HOURS OF POSITION and WORK CONDITIONS: The schedule of this role is flexible based on client needs, office hours during typical first shift, Monday-Friday will be common but applicants must be willing to work some evenings and weekends as dictated by the demands of the position, including client appointments, admissions, on-call coverage, and on-campus support.
Work locations range from human services agencies, clinics, hospitals, or offices, and often involve visits to client’s homes.
May involve contact with clients and/or family members who may be hostile, resistant, and violent.
PHYSICAL DEMANDS - Light Position may require lifting to 20 pounds at a time with frequent lifting or carrying of objects weighing up to 10 pounds.
Frequent walking, standing, and sitting.
Frequent reaching and/or grasping using hands and/or arms.
May be required to ascend/descend stairs.
Frequent use of hands requiring dexterity in using the telephone, computer keyboard, or other objects.
Visual ability including distance, peripheral and depth perception.
Ability to drive.
Regularly required to talk and hear.
