Responsibilities and Duties: 1.
Administer medications as prescribed by healthcare providers 2.
Monitor and assess residents' health status 3.
Implement and adjust care plans based on residents' needs 4.
Collaborate with physicians, nurses, and other healthcare professionals 5.
Provide emotional support to residents and their families 6.
Ensure proper documentation of residents' medical records 7.
Assist with residents' activities of daily living 8.
Maintain a clean and safe environment for residents Qualifications and Skills: 1.
Valid LPN license in the state of practice 2.
Minimum of 1 year of experience in long term care setting 3.
Knowledge of long term care regulations and standards 4.
Strong communication and interpersonal skills 5.
